I have recently received the Medical Death Certificate of Anthony Schultz. On it it states that LYNN JABLONSKI was the informant to his death. Anthony apparently resided at death from the Monticello Convelescent Center in Chicago. I am wondering if Lynn Jablonski was just a nurse or if there was another connection to Anthony... The certificate states that the relationship between Anthony and Lynn was "medical Records". Can anyone give me a clue??
